Can't Decide?  Here's a tip:

Use the FOM.xls if you would like to type in your selections and prices and have the shipping and tax figured for you.

Use the FOM.doc if you would prefer to figure the amounts yourself.

Either form may be returned to me via e-mail at heather@feltboardstories.com  by editing the document then attaching it to an e-mail.  For your own privacy and protection, please do NOT send payment information by e-mail, simply leave it blank and I'll contact you later for that info.

If you prefer to print out the form and fill it in by hand, you can use either form and then fax it to me at 1-770-252-8847 or mail it to me.
